Application Process

Thank you for choosing the Academy for your child's education.  To begin the admission process a registration form and a non refundable registration fee must be received by the Academy in order to reserve a space for your child.  

For your convenience an admission checklist and required forms are provided below in order to assist you in collection of the necessary documents and forms. Please note that you may type information directly into the forms below, then print them directly from the web page, or save them to your computer as PDF files to be printed later. To save the completed form to your computer, you must select the "print" feature, then select "Print to PDF."

Automatic Debit Forms (if applicable) are available in the Front Office. 

Once you have completed the above listed forms, please email or fax them to the Academy.  Forms can be emailed to and faxed to (513)755-0179.  Fees can be processed via credit card or by mailing a check to the Academy.  Please contact the office at (513)755-0169 for further information on payment processing.
